Our self-referral pages offer you an easy way to send your details to your local Social Services, so that you can obtain an assessment of your needs from an Occupational Therapist.  If you are not sure what you need, you can first carry out our self-assessment, which will suggest products that may be of use to you.

Why should I use it?

You can get the equipment you need in a number of different ways. You can, for example, simply buy it yourself. But you can also ask for help from Social Services.  

Our self-referral service can help you to contact your local Social Services. All you have to do is complete the self-referral form and send it to them through our secure link.  Social Services will then contact you directly to discuss your needs. We will give you a unique reference number to help you track your referral with them.  Your referral reference number and the date you self-referred will be saved in your "My equipmentdirect.org.uk" area for easy reference later.

Note that the self-referral service is not available in all areas. Before you complete the self-referral form, we will ask you to enter your postcode so that we can let you know whether you can take advantage of the service.

Do I need to self-assess?

Although you don’t need to complete the self-assessment questionnaire in order to use our referral service, it can help to speed up the process with Social Services. When you complete the self-assessment questionnaire, we record the questions you answer and your responses, and we can pass this information on to Social Services (though only if you want us to). If we do pass the information on, Social Services will already know something about you before they make contact.

Is it safe to send my details over the Internet?

We take your privacy very seriously, and have designed this service to be as secure as possible.  We use SSL (secure sockets layer) encryption to protect your identity. This ensures that your information can be read only by someone who is authorised to do so.  This system is used by all major banks for online transactions.
