Self assessment

If you are not sure what equipment is available or what would be best to make your day-to-day living easier, try our self assessment questions. They will help you identify your needs and suggest options for you to choose from.  To get the best out of this, you need to answer the questions realistically so that we can identify your need and suggest options to make your day to day living easier.  

At the end you will see a detailed confidential report.  This will contain suggestions for equipment, and also hints and tips which may help you. You can print the report and send it to a friend.  Your report will be saved for you so that you can return to it later, but don't worry, all the information held by us is confidential and only you can choose to share it. Self-referral

Do you feel that you (or someone you know) would benefit from an assessment by an occupational therapist (OT) from your Social Services department?  Why not try our self-referral service?  It's an easy way to send your details to your local Social Care Services department and you can even attach your completed self assessment report to reduce the number of questions you will have to answer. All information held by us is confidential and only you can choose to share your information.
