Child Tax Credit
HM Revenue and Customs has closed its on-line portal for Claiming Child Tax Credits so it is not possible to download any application forms.
If you want to claim by post, you can get an application pack and guidance notes from the Tax Credits helpline on 0845 300 3900 (textphone 0845 300 3909). The helpline is open seven days a week, from 8.00 am to 8.00 pm.
You can also get application packs for Working Tax Credit and Child Tax Credit from your local HMRC Enquiry Centre, social security office, Jobcentre or Jobcentre Plus office.
Christmas Bonus
You don’t need to claim Christmas Bonus – you'll get it automatically. If you think you qualify for the payment but haven’t got it, speak to the Jobcentre Plus or pension centre that deals with your payments.
Free Milk for Disabled Children
You can ask for claim form FW20 when you contact the Disability Benefits Unit.
When you complete and return form FW20, the Disability Benefits Unit might contact your doctor or hospital doctor who diagnosed your child's physical or mental disability.

Free Milk, Formula and Vitamins
If you’re pregnant
You can apply for free milk once you’re 10 weeks pregnant. It’s important to apply as soon as possible because milk vouchers can’t be backdated.
You can claim directly from the Department of Health’s Token Distribution Unit (TDU). You’ll need to complete a claim form and have your doctor or midwife fill in their part of the form. They’ll also need to provide evidence of the baby’s expected due date.

Statutory Adoption Pay
To claim Statutory Adoption Pay you must give your employer documentary evidence from the adoption agency. This will usually be a ‘matching certificate’.
You must also tell your employer when you intend to take leave within seven days of being told by the adoption agency that you've been matched with a child.
Statutory Maternity Pay
To claim Statutory Maternity Pay you must tell your employer at least 28 days before you intend to stop work to have your baby. Your employer may need you to tell them in writing.
You can change your mind about the date you want to start your Statutory Maternity Pay but you must still give your employer at least 28 days notice of the new date.
You must also give your employer evidence of when your baby's due. This is normally on maternity certificate MATB1 that your doctor or midwife will issue 20 weeks before your baby is due.
Statutory Paternity Pay
To claim Statutory Paternity Pay you must tell your employer when you intend to take leave by the 15th week before your baby is due, or within seven days of your wife, partner or civil partner being told by the adoption agency that they've been matched with a child.
If you change your mind you must give 28 days notice.
Your employer might ask you for self-certificate form SC3 becoming a parent that confirms you're entitled to the leave.
