Each form comes with notes that will help you fill it in and tell you where to send it. In either case, the date of the claim will be established as the date on which it is received by your local office.

Download a claim form to complete on your computer
You can complete a claim form on your computer for a person aged 16 or over from the Department for Work and Pensions website.
We recommend that you save the form on your computer before completing it: to do this simply right click with your mouse on the link below and choose the 'Save Target As' option. (You cannot save the form once you’ve opened it in an internet browser.)
Once you’ve completed the form, you can print and sign it. (Please note that the on-screen forms do not apply in Northern Ireland).

Free Milk for Disabled Children
You can ask for claim form FW20 when you contact the Disability Benefits Unit.
When you complete and return form FW20, the Disability Benefits Unit might contact your doctor or hospital doctor who diagnosed your child's physical or mental disability.

Vehicle Excise Duty (Car Tax) Exemption
Step one: get an exemption certificate
If you’re getting Disability Living Allowance, you’ll need exemption certificate DLA 404 from the Disability Benefits Unit on 0845 712 3456, textphone 0845 722 44 33 (7.30 am to 6.30 pm Monday to Friday).
If you’re getting the War Pensioner’s Mobility Supplement, you’ll need exemption certificate WPA 442 from the Veterans Agency on 0800 169 2277 (8.15 am to 5.15 pm Monday to Thursday, and 8.15 am to 4.15 pm on Fridays).
If you have a nominated driver, you must ask for their name to appear on the exemption certificate as well.
If you have an adapted car for a disabled person, you don’t need an exemption certificate.
Note: From 1st January 2006, exemption certificates will no longer be issued to Motability contract hire customers. This is because Motability dealers will arrange to register and license contract hire vehicles for you. Motability will hold the vehicle’s Registration Certificate (V5C) for the duration of the contract. When your vehicle tax is due for renewal each year they will send the new tax disc directly to you. It is therefore important that you let Motability know when you change your address.
Step two: use your exemption certificate to apply for a free tax disc
If you’re buying a new car, you can apply for a free tax disc through the dealer. You’ll need to give them your exemption certificate but make sure you get it back afterwards.
If you already have the vehicle’s registration certificate V5C, you can apply for a free disc at the Post Office®. You’ll need to complete form V10 (Vehicle Licence Application) and take your MOT and insurance paperwork as well.
If you only have the ‘New Keepers’ part of the registration certificate V5C/2, you need to apply for a disc at your nearest Driver and Vehicle Licensing Agency (DVLA) Local Office. (The V5C/2 is only valid for two months from the date you bought the car).
You’ll need to take or send forms:
nV62 (Application Form for a Vehicle Registration Certificate V5C);
nV10 (Vehicle Licence Application);
nyour MOT; and
ninsurance certificate.
